For Professionals
Only
The CoHub accepts referrals from working partners, statutory teams and voluntary sector organisations supporting Hackney residents.
If you are a professional seeking early intervention, practical support or structured community pathways for a resident - you can refer directly to us using the link below.
We provide responsive, place-based support that reduces escalation and connects residents into a trusted local infrastructure.
Who Can Refer?
We accept referrals from:
- Local authority teams
- Housing providers
- NHS and health partners
- Social prescribers
- Voluntary and community organisations
- Support workers and keyworkers
If you are unsure whether your referral is appropriate, please contact us before submitting.
What Makes Our Model Different
Referrals into the CoHub do not enter a waiting list in isolation. They are connected into:
- A trusted physical space
- Volunteer-led pathways
- Structured IAG appointments
- Co-production opportunities
- Community activity timetables
This reduces repeat referrals and improves sustained engagement.
Residents Can Be Referred For:
Professionals may refer residents for one or more of the following:
- Volunteering
- Leading an activity, workshop or community project
- Participating in focus groups or community research
- Attending activities or events at the CoHub
- Benefits, Council Tax and debt support (IAG)
- Accessing The Community Closet (clothing & cost-of-living support)
- Digital inclusion support (online access, forms, skills)
- Wellbeing, arts, crafts or gardening projects
- Skills training or personal development opportunities
- Clothing, food, fuel or digital support
Our approach is holistic. Many residents may require more than one type of support - this can be indicated on the referral form.
